Please state the number of people who want to apply (including the ages of any minors) and the days and times that are most convenient for appointments, and your phone number. Each applicant must bring the DS-11 form, proof of citizenship, proof of identity, a passport photo (which can be taken by the Library for $10), separate checks or money orders for each applicant to the U.S. Department of State for the passport fees, and one check or money order for the execution fees ($35 per person) and photo fees ($10 per person) to the Baldwin Public Library.
Photo service is available for applicants two and older.
Agents may limit the number of applicants they see at one time to carry out their other Library responsibilities.
Our service does not include adult renewals (which must be done by mail with form DS-82), but we can review your DS-82 form and passport, and take a new photo for $10.
